What is it?
The Office of Institutional Effectiveness collects, analyzes and reports data to support institutional decision-making and strategic planning. If your office requires institutional data for assessment, accreditation, compliance or other reporting needs, please use this form to initiate a data request. A member of the Institutional Effectiveness staff will work with you to generate the information you need.

How Do I Use It?
Fill out the form with as much detail as possible. If you have a template or reporting form that needs to be used, please attach it to the request or provide a link.
Please note that most data requests require approximately 1-2 weeks to complete. If the request is needed sooner, please complete the form and follow up with the Office of Institutional Effectiveness at institutionaleffectiveness@usj.edu.
