This form is for IT related project ideas. Please fill the form out in it’s entirety. It will then be routed to the IT Director / CISO who will determine what area of IT would be responsible for this project and assign it to them to determine the amount of effort and resources it would require from the IT side. Once completed they will assign it back to the IT Director / CISO to be brought before the IT Ops Committee to be adopted and prioritized. The project will not be considered adopted and active until all these steps are completed and an IT resource is assigned to begin working on it.
